Privacy at a glance
- We collect what we need to fill your order: contact, billing and shipping details, the text and artwork you want printed, and (for checks) the bank information required to print them.
- We do not sell your personal information for money, and we do not sell customer lists.
- We share information only as needed with the print houses that produce your order, payment processors, shipping carriers and the service providers that run this website.
- We never see or store your full card number. Card and wallet payments are handled by PayPal and WooPayments (Stripe).
- You are in control. You can ask to access, correct or delete your information, unsubscribe from email at any time, and manage cookies from our cookie banner.
Who we are and what this policy covers
Form Technology LLC (“Form Technology,” “we,” “us” or “our”) is a business forms and printed supplies company that has served customers since 1992. We sell products through our website and work with national print houses across the United States to produce and deliver your order.
This policy applies to information collected through www.formtechnologyco.com, including our online product pages, live proof and preview tools, quote requests, shopping cart and checkout, customer accounts, contact forms, and email, phone and chat communications with our team. It does not apply to third-party sites we link to.
Information we collect
Information you give us
- Contact and account details: name, company name, email address, phone number, and password if you create an account.
- Order details: billing and shipping addresses, products and quantities, order notes, coupon codes, and tax-exemption information you provide.
- Personalization content: the text, names, addresses, logos, images, signatures and artwork you enter or upload to have printed on business forms, envelopes, note pads, labels, invoices, checks and promotional products.
- Check-printing information: for check orders, the business or account-holder name and address, bank name, bank routing (ABA) number, account number, starting check number, and any signature or logo you supply. This is the information required to print MICR-encoded checks that your bank will accept.
- Tax-form and imprint information: payer names, addresses and identification details you ask us to imprint on personalized forms and kits. Please provide only what is needed for the product you are ordering.
- Quote requests, forms and messages: anything you submit through our quote, contact or other forms, or send us by email, phone or chat, including file attachments.
- Reviews and feedback: ratings, comments and survey responses you choose to provide.
Information collected automatically
- Device and browser type, operating system, screen size, and language.
- IP address and the general (city or region level) location derived from it.
- Pages viewed, products and proofs viewed, links clicked, referring website, search terms used on our site, and the date and time of your visit.
- Cookie and similar-technology identifiers, described in the Cookies section.
Information from other sources
We receive limited information from payment processors (for example, confirmation that a payment was authorized and fraud-screening results), shipping carriers (delivery status), and analytics and advertising providers (aggregated performance data). We may also receive your business contact information if you are referred to us by a colleague or one of our print partners.
Artwork, logos and online proofs
Form Technology lets you add your text or upload artwork and see a live preview or proof before you buy. Here is how that content is handled:
- Purpose. Files and text you enter are used to generate your on-screen proof, prepare your order for production, verify artwork with you when needed, and reprint or reorder your items.
- Sharing. To produce your order, your artwork and imprint text are sent to the print house or manufacturing partner that fills it. Those partners may use it only to produce and ship your order.
- Your responsibility. Upload only artwork, logos, trademarks and text that you own or are authorized to use. Please do not upload files containing sensitive personal information (such as Social Security numbers or full financial-account numbers) unless the product requires it, and never upload files with hidden or embedded personal data you do not want printed.
- Storage. Uploaded files are stored on our web hosting and backup systems and may be copied to the cloud file-storage and order-management tools we use to process orders. Files uploaded through our forms are visible only to our team and the partners who need them. We keep uploaded images and artwork for up to two years, and we may ask you to send updated images at any time.
- Removal. You may ask us to delete your saved artwork at any time (see Your privacy choices), subject to any order still in production and any records we must keep by law.
Payment information
We accept payment through PayPal (including PayPal, Venmo and Pay Later where offered, and cards and wallets processed by PayPal) and WooPayments, which is powered by Stripe (cards, Apple Pay and Google Pay). When you pay, your card or wallet details are entered directly into the processor’s secure fields or window and are processed by them under their own privacy policies. We do not receive or store your full card number or security code. We receive limited details such as the payment method type, the last four digits, the billing address, and the transaction status so we can record and support your order.
Processor privacy policies: PayPal and Stripe.
How we use your information
- To process, produce, ship, track and support your orders, including sending order confirmations, proofs, invoices, packing slips and shipping updates.
- To calculate sales tax and shipping, and to detect and prevent fraud and unauthorized transactions.
- To create and manage your customer account, wish lists and saved information.
- To respond to quote requests, questions and support tickets.
- To send you marketing emails and promotions about business forms, checks, envelopes and related products, where you have signed up or where the law allows.
- To operate, secure, troubleshoot and improve our website, live proof tools, product catalog and customer service.
- To measure and improve our advertising and to show our products in Google search and shopping results.
- To comply with tax, accounting and other legal obligations, enforce our terms, and protect our rights, customers and property.

Cookies, analytics and advertising
We use cookies and similar technologies to keep your cart and login working, remember your preferences, protect the site from abuse, and understand how visitors use it. Our cookie banner (powered by WPConsent) lets you accept, decline or customize non-essential cookies, and you can change your choice at any time. A detailed list of cookies is in our Cookie Policy.
- Strictly necessary cookies support the shopping cart, checkout, security and login. They cannot be turned off because the site would not work without them.
- Preference cookies remember choices such as your consent settings and saved form details.
- Analytics and advertising cookies (such as those from Google) help us measure traffic, understand which products and pages are useful, and measure advertising. These are used only according to your cookie choices where the law requires consent.
You can also block or delete cookies in your browser settings, though some parts of the site may stop working. Where required by law, we treat a browser-based opt-out preference signal such as Global Privacy Control as a request to opt out of the sale or sharing of personal information. Because there is not yet a common standard, our site does not respond to “Do Not Track” browser headers.
Google’s use of information collected through Google for WooCommerce is governed by the Google Privacy Policy. You can manage Google ad personalization at adssettings.google.com.
Email and marketing communications
If you subscribe to our email list, place an order, or ask for a quote, we may send you product news and promotions. Every marketing email includes an unsubscribe link, and you can also ask us to remove you at any time using the details in the Contact section. We comply with the CAN-SPAM Act. Even if you unsubscribe from marketing, we will still send transactional messages such as order confirmations, proofs, invoices and shipping notices. We do not sell or rent our email list.
How long we keep information
- Order, invoice and tax records: generally up to seven years, to meet accounting, tax and legal requirements.
- Artwork, logos, images and proofs: kept for up to two years so we can reprint and reorder for you, then deleted. We may contact you at any time to request updated artwork or images, and you can ask us to delete them sooner.
- Bank information for check orders: kept only as long as needed to fill the order and to make reorders accurate, and deleted on request, subject to legal record-keeping requirements.
- Customer accounts: until you close the account or ask us to delete it.
- Quote requests and support messages: generally up to three years.
- Marketing preferences: until you unsubscribe; we keep a minimal suppression record so we do not email you again.
- Website logs, comments and analytics data: for as long as needed for security and site operation; comments and their metadata are retained so follow-up comments can be recognized.
When information is no longer needed, we delete or de-identify it. Backups roll off on a regular schedule.
How we protect your information
We use reasonable administrative, technical and physical safeguards, including HTTPS encryption of data in transit, security headers, spam and bot protection, access controls, regular software updates, malware and vulnerability scanning, and encrypted payment processing by PCI-compliant providers. Access to customer and artwork files is limited to team members and partners who need it to fulfill your order. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. If we become aware of a breach affecting your personal information, we will notify you and regulators as required by law.
Your privacy choices and rights
Depending on where you live, you may have some or all of the following rights. We offer them to all customers as a matter of policy:
- Know and access the personal information we hold about you, including a portable copy.
- Correct inaccurate information.
- Delete your personal information, subject to legal exceptions (for example, tax records and orders in production).
- Opt out of the sale or sharing of personal information, targeted advertising, and profiling; use the cookie banner settings or contact us.
- Limit our use of sensitive personal information; we use it only to provide the products and services you request.
- Unsubscribe from marketing email at any time.
- Appeal a decision about your request; if you are in a state that provides an appeal right, reply to our decision or contact us with the word “Appeal” in your message.
- Non-discrimination: we will not deny you service, charge you a different price, or provide a different level of service because you exercised a privacy right.
To make a request, use our Contact Us page and include “Privacy Request” in the subject, call us at 479-855-3562, or write to us at Form Technology LLC, P.O. Box 389, Springdale, AR 72765. Tell us which right you want to use and the email address or order number connected to your information.
We may need to verify your identity, for example by matching details to an existing order or account, before acting on a request. You may use an authorized agent; we may ask for written proof of their authority. We respond within 45 days, and if we need more time we will tell you why and may extend once by up to 45 more days.
